WAIVER AND CONSENT

I, the parent or legal guardian, hereby give permission for my child, to

participate in the 2025 training sessions organized and operated by Next Generation Hoops NC and

Isaiah Maurice upon communicating, and parti all activities. These sessions are held weekly and may take place on any day throughout the

calendar year 2025, at various locations spread across North Carolina.

Waiver of Liability:

I understand and acknowledge that participation in sports training activities involves inherent risks,

including but not limited to physical injury, illness, and property damage. I hereby release and hold

harmless Next Generation Hoops NC, its staff, volunteers, and affiliates, as well as Isaiah Maurice,

from any and all liability, claims, demands, causes of action, or expenses arising from or related to

any injury, illness, or other incident that may occur while my child is participating in any training

session or event.

I understand that this waiver applies to all sessions held in the year 2025, regardless of frequency,

schedule, or location, and includes travel to and from any training event at any of our North Carolina

locations.Payment as Consent:

I understand and agree that any payment made to Next Generation Hoops NC constitutes my full

and voluntary agreement to the terms of this waiver and liability release. Payment also signifies my

acceptance of all rules, regulations, and policies of Next Generation Hoops NC. No signature shall

be required in addition to payment to affirm this consent.

Medical Treatment Consent:

In the event of a medical emergency, I authorize the staff of Next Generation Hoops NC and Isaiah

Maurice to seek appropriate medical care for my child. I understand that I will be notified as soon as

possible should such care be necessary.
